Phaxis is looking for a qualified *Contract School Based Occupational Therapist *to join our team in Schools in the Hartsville, SC area for the remainder of the 2024-2025 Academic Year!

*Details:*
* Position: Contract School Occupational Therapist in the Hartsville, SC area
* Duration: Remainder of the 24/25 School Year
* Hours: Full Time Position, Regular School Hours (Mon-Fri)
* Caseload: K-12 (multiple positions available)
*Requirements:*
* OT State Licensure
*Responsibilities*
* Assess students' needs and develop individualized treatment plans to enhance their functional abilities.
* Provide therapeutic interventions to support students in performing daily activities and achieving educational goals.
* Collaborate with teachers, parents, and other professionals to create a supportive learning environment.
* Monitor and document progress, adjusting care plans as necessary.
* Educate staff and families on strategies to assist students with disabilities.
* Participate in discharge planning and transition services for students moving to new educational settings.
