*Position: Home Health Occupational Therapist (OT)*
We're looking for a dedicated full-time OT to provide personalized, one-on-one care to patients in their homes. This role offers the autonomy to create tailored treatment plans and enhance patients' independence and quality of life. Enjoy flexible scheduling!
*What You'll Do:*
* *Empower Daily Living: *Conduct thorough patient evaluations and develop individualized treatment plans based on physician orders.
* *Restore Independence: *Assist patients in improving daily living activities and functional mobility through targeted interventions.
* *Optimize Home Environments: *Provide hands-on therapy, including adaptive equipment training and home modification recommendations, to enhance safety and accessibility.
* *Educate and Support: *Provide comprehensive education to patients and families on treatment procedures and home exercise programs.
* *Drive Progress: *Continuously reassess and modify treatment plans to maximize patient outcomes.
* *Collaborate for Success: *Work closely with physicians and a multidisciplinary team to ensure seamless, coordinated care.
* *Maintain Excellence: *Ensure accurate and timely documentation using Epic EMR.
*What You'll Bring:*
* Bachelor’s Degree in Occupational Therapy (Required).
* Active Illinois Occupational Therapist License in good standing (Required).
* Valid Driver’s License (Required).
* A passion for home health and a patient-centered approach.
* Occupational Therapists with a passion for home health are encouraged to apply—*training available!*
*Schedule & Flexibility:*
* Monday – Friday, (8:00 AM – 4:30 PM) with occasional weekend and holiday rotation.
* *4-10s available!*
* Centralized territories to minimize travel.
*Compensation & Benefits:*
* Competitive Salary: $65,000 - $110,000 per year (commensurate with experience).
* *$2,500 Sign-On Bonus! *(Paid in the 2nd pay period).
