SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Paid Mileage
MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile – one of the highest in the industry!

REWARDING WORK YOU WILL DO:
Provide initial and ongoing assessments and implements strategies including Motivational Interviewing techniques to conduct a semi-structured client interview to identify and prioritize problem areas.
Capture the goals of the participant in up to 3 Action Plans, creates a Work Order and coordinates with the construction specialist.
Discuss with the participant and observes his/her functional performance for safety, efficient and difficulty and accesses the presence of environmental barriers for each targeted area.
Develop a prioritized work order to include home modifications, repairs, and necessary items that will help the participants meet their goals.
Reinforce strategy use, and review problem solving, refined strategy use, and provide education and resources to address future needs for environmental adjustments in the final sessions with clients
Collaborates across disciplines involved with care of the client, including but not limited to the CAPABLE RN and the handy person/construction specialist.
WHAT WE WILL LOVE ABOUT YOU:
Minimum Education: Graduation from a college or university, with a degree in Occupational Therapy.
Minimum Experience: One year of experience as an occupational therapist required, with two years of SNF, hospital or inpatient rehab preferable. Home health experience also preferred. Other requirements may vary by case or department needs.
Required License: Active Colorado OT license. Current CPR certification. CPR must be provided and/or obtained. Valid Colorado driver’s license and proof of automobile insurance in accordance with state statute.
PHYSICAL REQUIREMENTS:
Ability to lift/push/pull minimum of 35 lbs
