*Responsibilities:*
* Evaluate patients and develop personalized occupational therapy treatment plans.
* Implement therapy interventions to improve activities of daily living (ADLs), mobility, strength, and functional independence.
* Educate patients and caregivers on strategies for improving self-care, home safety, and functional tasks.
* Maintain accurate documentation and patient records.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care in the SNF setting.
*Qualifications:*
* Master’s or Doctorate degree in Occupational Therapy from an accredited program.
* Current state licensure as an Occupational Therapist (OT) in *TX*
* Previous experience in a Long-Term Acute Care (LTAC) setting is preferred but not required.
* Strong communication and interpersonal skills.
* Ability to work effectively in a team-oriented environment.
* Passion for delivering excellent patient care.
