* Collaborate with an interdisciplinary care team to support holistic, person-centered care

* Support discharge planning and ensure continuity of care through home safety evaluations and care coordination
* Participate in quality improvement, staff development, and peer training programs
* Maintain a clean, safe environment and follow all safety protocols and regulatory standards
*Occupational Therapist Required Qualifications:*
* Graduate of an *accredited Occupational Therapy program*
* *New York State licensure as an Occupational Therapist*
* At least *1 year of experience* as an Occupational Therapist preferred
