Occupational Therapist - OT
Summary:
We are looking for a committed and patient-focused Per Diem Occupational Therapist (OT) to work for our home care department. This person will develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. You will offer effective and client-centered services that will enable people to carry out everyday tasks or activities confidently and independently.
This is a field-based position.
Responsibilities:
Provide exceptional Occupational Therapy (OT) to patients by assessing and understanding the care plan.
Evaluate patient's functional status and home environment for hazards or barriers to be independent living and identify any equipment needs.
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
Participate in the development of the individualized plan of care.
Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
Submits all clinical documentation, time records and other information in the required time frame.
Supports company mission by providing the highest quality of home care.
Qualifications:
Licensed Occupational Therapist in the state of Connecticut with at least 1 year of clinical experience; Home Care preferred.
Oasis experience strongly desired.
Have a valid driver’s license, reliable transportation and willing to travel throughout an assigned territory.
Ability to work independently with minimal supervision.
Great communication, organizational and time management skills.
Ability to solve problems.
Have great interpersonal skills.
Excellent computer skills.
