Responsibilities:
We’re looking for Occupational Therapists to join our Home Health team that serves the special purpose of caring for Valley patients in the safest place to them, their home. In this role, you will provide occupational therapy services to patients, focusing on assessment, treatment, program planning, and implementation to help patients achieve optimal functional independence. You will collaborate with a multifaceted team of seasoned therapists, and mid-career clinicians, averaging 25-30 visits per week and establishing individualized plans of care.

You will work within an interdisciplinary team caring for patients with conditions such as Stroke, Traumatic Brain Injury, Spinal Cord Injury, Cardiac, Orthopedic, and more. Qualifications:
Minimum Requirements, Licenses and Certifications
Bachelor's degree in Occupational Therapy
Completion of internship program in Occupational Therapy required
State of California Occupational Therapy license or eligible for licensure
DL - Valid California Driver's License and driving record acceptable to Community Health System required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
