The Occupational Therapist will assess functional activity demands against the performance ability of the client. They will identify and address deficits to improve the client’s quality of life and rebuild or maintain the client’s independence. Flexible scheduling allows employees to work as many or as little hours as they want as our team is looking for high-energy individuals for part-time work.

*Responsibilities*
* Work cooperatively with physicians to develop an appropriate treatment and care plan specific to the client and their physicians report
* Assess the home environment, especially as it related to fall risk and intervention and home safety
* Identify equipment needs and develop adaptive plan to improve client function and independence
* Report client’s response to treatment or changes in condition to the Director of Nursing and physician
* As appropriate, coordinate interventions with other health care providers and live setting such as worksites, adult day care or residential setting while instructing appropriate personnel, family and caregivers, on intervention goals
* Prepare clinical and progress summaries as well as discharge planning. Submit documents to Director of Nursing
* Treat clients, family, and staff with courtesy and respect
*Requirements*
* Graduated from an occupational therapy program accredited by the Accreditation council for Occupational Therapy Education (ACOTE) or predecessor organization
* 1+ year experience in Home Health Care
* CPR
* Actual TB
* Reliable Transportation
* Clean background check
* Pass a 10 panel drug screen
* State licensure in state where care is being provided
* Abide by Occupational Therapy Code of Ethics (AOTA, 2005a) and demonstrated knowledge about and will deliver occupational therapy services in accordance with AOTA standards
* Adhere to HIPAA and maintain client confidentiality
