* *Location*: Durham
* *Focus*: Frailty-specialist OT working in primary care settings.
* *Goal*: Help older adults stay well at home, avoid hospital admissions, and support faster discharges.
*Key Responsibilities*
*Clinical Practice*
* Deliver biopsychosocial OT interventions from assessment to evaluation.
* Use frailty indexes and risk stratification tools.
* Provide rapid crisis response and short-term rehab.
* Collaborate with MDTs, patients, carers, and social workers.
*Evidence & Development*
* Apply clinical governance, audits, and HEE’s Roadmap to Practice.
* Support discharge planning and environmental adaptations.
* Develop SOPs and contribute to service pathway improvements.
*Leadership*
* Lead OT service development across PCNs.
* Promote cross-sector collaboration and best practice.
* Act as a thought leader in primary care OT.
*Learning & Mentorship*
* Facilitate interprofessional education.
* Mentor colleagues and deliver in-house training.
* Drive audit-informed service improvements.
*Skills & Attributes*
* Strong clinical reasoning, communication, and adaptability.
* Tech-savvy with modern digital tools.
* Committed to CPD and reflective practice.
* Patient-centred decision-making aligned with NICE guidance.
*Qualifications*
* BSc or pre-reg MSc in Occupational Therapy (RCOT approved).
* HCPC & RCOT registered.
* Verified HEE Roadmap Stage 1.
* MSc in Advanced Practice (desirable).
