*Key Responsibilities*
* Assess patients in their homes to determine functional deficits and rehabilitation needs
* Create individualized care plans to meet therapeutic goals
* Deliver direct OT services using evidence-based practices

* Supervise support staff where applicable
* Maintain accurate and timely clinical documentation
*Requirements*
* Active state-issued license in Occupational Therapy
* Valid driver’s license and reliable transportation
* Comfort working autonomously in home settings
* Strong interpersonal and critical-thinking skills
*Preferred Qualifications*
* Previous home health, acute care, or rehab experience
* Proficiency with mobile EMR systems
