A health services system in New York is looking for a licensed Occupational Therapist (OT) to join their team throughout Queens.
About the Opportunity:
Schedule: Monday to Friday
Hours: 9am to 5pm (1 hour lunch)
Setting: Hybrid (Clinic and Field)
Responsibilities:
Assess individuals referred by healthcare providers and implement treatment plans collaboratively
Complete required documentation such as assessments, progress updates, treatment notes, discharge summaries, and interdisciplinary notes in accordance with organizational policies
Deliver evaluations and therapeutic interventions at designated locations, including clinical sites, patient homes, and care facilities
Ensure that the environment is maintained in a clean and safe condition, adhering to all safety protocols
Participate as an active member of the interdisciplinary care team to help develop and revise individualized care plans in compliance with applicable regulations
Record relevant participant information including evaluations, progress notes, and care planning documentation, in accordance with established procedures
Maintain and report statistical and operational data as required
Participate in quality assurance and performance improvement activities
Perform other duties, as needed
Qualifications:
Completion of an accredited Occupational Therapy program
Bachelor's Degree
Proficiency in the use of Occupational Therapy modalities and related equipment
Capability to travel to off-site locations, including patient residences and affiliated facilities
Desired Skills:
1+ year of Clinical experience in Occupational Therapy
Experience working with the Geriatric population
Familiarity with regulations related to Elder Care services
Experience with Electronic Medical Records systems (such as Care Compass)
