Occupational Therapist, OT Job Description:

The Occupational Therapist (OT) specializes in improving residents' daily living skills and independence in our skilled nursing facility.

Occupational Therapist, OT Responsibilities:

Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.
Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments.
Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
Documentation: Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.

Occupational Therapist, OT Qualifications:

State licensure as a Physical Therapist.
