Minimum Qualifications:

Must be licensed as an Occupational Therapist in the state of Oregon
Graduate from an accredited program with a degree in Occupational Therapy
CPR Certification
National Provider Identifier (NPI) required upon hire

Preferred Qualifications:

Experience in Home Health
Organizational skills to facilitate program development

Responsibilities:

Establish treatment plan appropriate to physician orders
Prepare/complete clinical and progress notes
Teach patient and family/caregivers in meeting individual treatment goals
