* Evaluate patients and develop personalized treatment plans to improve functional independence.
* Deliver occupational therapy services tailored to each patient’s needs.
* Educate patients and caregivers on therapy techniques, safety, and continued care.
* Document all care provided in accordance with state and facility guidelines.
* Collaborate with interdisciplinary team members to achieve optimal outcomes.
*Qualifications*
* Active Pennsylvania OT license (or eligibility).
* SNF experience preferred but not required.
* Strong communication and interpersonal skills.
* Commitment to high-quality, patient-centered care.
