Occupational Therapist

*Job Summary*
We are seeking a compassionate and dedicated Occupational Therapist to join our team. The ideal candidate will have a strong background in providing therapeutic services to individuals across various settings, including pediatrics, geriatrics, and skilled nursing facilities. This role involves developing and implementing personalized care plans, conducting assessments, and collaborating with multidisciplinary teams to enhance patient outcomes.

*Responsibilities*
* Conduct comprehensive evaluations to assess patients' needs and develop individualized care plans.
* Implement therapeutic interventions to improve patients' functional abilities and quality of life.
* Collaborate with healthcare professionals to coordinate discharge planning and ensure continuity of care.
* Provide case management services, including documentation of progress and modifications to care plans as necessary.
* Work with diverse populations, including individuals with autism, in both inpatient and home health settings.
* Educate patients and their families about treatment strategies and self-care techniques.
* Participate in early intervention programs for children to promote developmental skills.
* Maintain accurate records of patient progress and treatment outcomes in compliance with regulatory standards.

*Qualifications*
* Master's degree or Doctorate in Occupational Therapy from an accredited program.
* Current state licensure or eligibility for licensure as an Occupational Therapist.
* Experience in geriatrics, pediatrics, or skilled nursing facilities is preferred.
* Knowledge of case management principles and discharge planning processes.
* Familiarity with therapeutic approaches for individuals with autism and other developmental disorders is a plus.
* Strong communication skills, both verbal and written, to effectively interact with patients, families, and healthcare team members.
* Ability to work independently as well as part of a collaborative team environment.
