Home Health Occupational Therapist Overview:

Your Role and Responsibilities as a Home Health Occupational Therapist:
Assist the physician in evaluating patient function, performance, and equipment needs
Develops a treatment plan with long-term goals, intervention, and service intensity
Participates in interdisciplinary patient assessments and plan development with the care team
Regularly reassess patient progress and modify the plan of care if necessary
Advises and educates patients, families, and caregivers to promote progress towards goals and discharge planning
Documents and communicates with the healthcare team to ensure coordinated care and planning for discharge
Qualifications for an Interim HealthCare® Occupational Therapist:
Graduate of an accredited Occupational Therapist Program with an active OT license in IL
Minimum 2 years of occupational therapy experience, ideally in home healthcare
Current CPR and BLS certifications
Strong clinical judgment, excellent communication skills, and a compassionate approach
