Purpose:
Provides comprehensive Occupational Therapy services to individuals in the home setting for the purpose of increasing and maximizing functional abilities to perform daily self-care tasks, accepting primary care responsibilities, and coordinating with other disciplines and community agencies.
Responsibilities:
Evaluates/assess the patient/client and determine a treatment plan using approved examination and evaluation procedures.
Interprets evaluation findings and communicate the treatment plan to the patient/client, family, caregiver, and care team.
Prepares necessary documentation including evaluations, daily notes, progress notes, updated plans of care, discharge summaries and logs in a timely manner according to federal, state and facility-specific rules and regulations.
Consults with other members of the rehabilitation team to select treatment programs consistent with the needs and capabilities of the patient and to coordinate comprehensive services delivery.
Develops effective home programs to maintain and enhance patient/client performance in his/her environment.
Develops treatment protocols/procedures using evidence-based guidelines, patient/client-specific goals, timely documentation, and discharge recommendations.
Develops and initiate referrals from licensed healthcare professionals. Respond in a timely manner to referrals.
Develops effective rapport with referring healthcare practitioners.
Presents a positive service-oriented approach with patients/clients, family, caregivers, facility staff, and co-workers at all times.
Monitors patient/client progress and modify the treatment plan and procedures as appropriate. Review the patient/client treatment program to maintain effective and efficient quality service.
Trains therapists in skills and techniques required for participation in activities and evaluate therapists’ functional progress.
Assists with orientation of new staff, volunteers, and students. Supervise occupational therapy assistants in accordance with federal and state regulations.
Assists with equipment maintenance and monitor supply inventory.
Demonstrates ongoing commitment to reducing costs and improving productivity in the rehabilitation department.
Demonstrates the ability to handle changing schedule of patients/clients.
Maintains positive working relationships with the healthcare team, patient/client, family, and caregiver.
Assists with Quality Assurance reports and records. Complete EveryStep and facility reports as required.
Assists with development of programs. Attend department meetings and conferences.
Understands and comply with EveryStep’s policies and procedures, including but not limited to, universal precautions, safety, risk, and emergency.
Qualifications:
Bachelor’s Degree in Occupational Therapy
Current and valid Iowa OT license
Ability to comply with accepted professional standards and ethical guidelines
Current Healthcare Provider CPR certification required
Mandatory Reporter of Child and Dependent Adult Abuse training certification (within 30 days of hire)
Must have a valid driver’s license, auto liability insurance, and reliable transportation
Successful completion of pre-employment physical, drug screen, 2-step TB test, and background check
Working Conditions:
Physical requirements: Medium work exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
Physical activities: Climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, talking, fingering (picking, pinching, typing, etc.), hearing, grasping (applying pressure with fingers and palm), repetitive motions, feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)
Environment: Extreme temperatures, Hazards (chemicals, infectious disease), atmospheric conditions (fumes, odors, dusts, mists, gases)
Time: Ability to be flexible with a varying schedule and programmatic requirements
