Qualifications:
Associates Degree in Occupational Therapy from an accredited program.
Successful completion of the National Certification for OTA.
Assumes responsibility for ongoing continuing education and professional development.
Current state licensure or license eligible.

As a Certified Occupational Therapy Assistant in a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to:

Essential Job Functions:
Provides occupational therapy services to residents under the direct supervision of the Occupational therapist.
Conducts screenings of residents at regular intervals to determine need for intervention/treatment.
Identifies indications for evaluations at appropriate intervals and assists in the evaluation under the direction of the
Occupational Therapist.
Assists the Occupational Therapist in the evaluation process in the areas of self-care, homemaking, functional mobility,
coordination and other areas as directed
Assists the Occupational therapist in identifying a target list, including short-term goals, long-term goals and treatment plan.
Conducts both individual and group treatment as assigned by the Occupational Therapist and outlined in treatment plan
that includes, but is not limited to, areas of ADL, functional mobility, ROM, strength, coordination, cognitive/perceptual
remediation and functional maintenance.
Assists in the selection of occupational therapy techniques/media and in the sequence of activities, adapts
techniques/media as indicated by the residents performance and following consultation with the Occupational Therapist.
No major modification to the treatment plan is made without prior consultation with the Occupational Therapist.
Monitors residents response to intervention and consults with the Occupational Therapist if modification is indicated..
Assists in establishing and modifying individual resident schedules in conjunction with the Occupational Therapist and
other team members.
Adheres to department and resident schedule and modifies as appropriate for treatment regimen.
Maintains acceptable resident care ratios as determined by the Occupational Therapist.
Provides, recommends and fabricates adaptive devices or other equipment. Trains resident, family and appropriate staff
in the use or application of such equipment.
Participates in resident, family and staff education.
Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
Adheres to Occupational Therapy Department procedures regarding documentation of occupational therapy services.
Provides treatment in accordance with established standards of practice, department procedures and productivity
standards.

Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of resident therapy programs.
Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
Assists with and participates in Quality Assurance Program as required.
Maintains a current knowledge of the law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are in compliance with applicable regulations.
Attends and participates in scheduled training, educational classes and meetings required or requested for position.
Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions and OSHA standards for the healthcare professional.
Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel immediately.
Responds to and acts appropriately in emergency or disaster situations.
Adheres to high standards of cleanliness, grooming, hygiene and dress code.
Adheres to all policies of company, including job description, certification requirements, mission statement, HIPAA, confidentiality standards and residents rights.
Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and in flexible in carrying out assignments.
Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control.
Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.
